【中文摘要】2009年4月7日发布的《医药卫生体制改革近期重点实施方案(2009—2011年)》。公立医院的运营环境发生了彻底改变：医疗市场放开,多种成分的医疗服务主体开始涌入,社会办民营专科医院、民营综合医院、中外合资合作医疗机构的迅速发展,各类性质的医院将对公立医院的生存和发展构成威胁。而在省会城市,大医院有省级医院,技术力量雄厚,小医院有区医院、厂矿医院和社区医院,政策灵活。但不同级别医院之间没有形成转诊关系,错位竞争,加剧了竞争。政府财政投入比例逐年减少,作为郑州市级公立医院的确是在夹缝中生存。加上各类医保制度的实施,推广参保人员就医“一卡通”,使得传统劳保、免费医疗的定点式医疗体系瓦解,郑州市级公立医院的生存和发展面临着比以往更加严峻的形势。基于郑州市级公立医院的公益性质和政府的有力规制,医院在资源配置的权力、渠道和市场化竞争手段(如价格)方面均受到很大限制,其服务耗费的补偿却主要来自医疗市场,加强战略管理日见必要,以提高医院竞争实力,适应日益严峻的生存环境。本文在概析医院发展战略内涵和基本特点的基础上,针对郑州市市级公立医院在发展过程中存在的问题和原因分析,综合运用非赢利组织理论、资源导向分析理论(SWOT分析法)、竞争导向分析理论和人力资本理论等工具,对郑州市市级公立医院发展的内外部环境进行了综合分析,提出了郑州市市级公立医院发展的战略目标和四种主要模式,即“信息化医疗集团中心”模式、“大专科、上

规模、信息化,强病房”模式、“大专科、小综合、信息化,差异化”模式和“大专科、信息化,大门诊”模式。并结合郑州市市级公立医院的发展实际,就战略模式选择问题提出了见解。作为对研究成果的运用,论文以郑州市第一人民医院为例,就郑州市市级公立医院发展战略模式的构建、实施进行了实证分析。

【英文摘要】April 7.2009《medical and health system innovation plan (2009-2011)》has issued. Public hospital operating environment has completely changed, therefore, the medical treatment has unlocked, different medical service has join to this environment. Private specialist hospitals, private general hospitals and the joint venture or cooperative medical institutions was rapid development in this time. Those medical institutions will be intimidated the public hospital’s exist and development. In the capital city, provincial hospital is as good as the technical force. District hospital and community hospital are as good as the policy flexibility. But different levels of hospital did not have the referral relationship between each other. So it leads the competition prick up. Government was decreasing the financial investment day by day. And Zhengzhou city public hospital is really hard to survival, coupled with various types of medical insurance system implementation makes this status harder and harder. Base

on the governmental prescription and commonweal character, public hospital must strengthen stratagem management and increase the compete strength to adapt the environment.Summary on the hospital development in this strategic nature and basic characteristics, based on the public hospital for Zhengzhou City in the development process according the problems and the integrated add the non-profit organization theory, theory of resource-oriented analysis (SWOT analysis),

competition-oriented theory and the theory of human resource. And I will be analysis the development and a comprehensive internal and external environment of Zhengzhou City public hospital. Then I will bring out the four major strategic objective model, that “information Medical Group Center model”, “the big specialist, on the scale, information technology mode”, “the big specialist, small comprehensive, information, and difference model” and” big specialist, information technology, large out-patient model”. I combined with the actual development of the Zhengzhou city public hospitals to put forward a strategic view mode selection. As the good example of Zhengzhou City No.1 People Hospital is use as research results, that to develop strategic model to building Zhengzhou city public hospital and analysis the

implementation of an empirical at last.
